Disparities in health outcomes and care in Wisconsin by race, ethnicity, and payer
WCHQ and its member organizations, in collaboration with HIP, developed the 2019 Wisconsin Health Disparities Report to identify where disparities in health outcomes and care exist in Wisconsin and to help inform and accelerate programs that are working to eliminate disparities. The poster 1. Identifies where substantial disparities in health outcomes and care exist in Wisconsin 2. Accelerate programs working to eliminate disparities in Wisconsin 3. Engage stakeholders of all backgrounds to work collectively to reduce disparities.
ICTR Neighborhood Health Partnerships Program: Timely, Local Data to Guide Action in Wisconsin
The ICTR Neighborhood Health Partnerships Program aims to advance the health of Wisconsin communities by reducing barriers to learn, act, and sustain improvements. The poster outlines the program’s goals: 1) Learning about the health of Wisconsin neighborhoods through timely and actionable health outcomes and care data, 2) Supporting report users in moving from data to action with a trained core of navigators to help users better understand the data; use action tools to select, adapt, and implement interventions; and create partnerships to develop, implement, and evaluate interventions, and 3) Monitoring outcomes and motivating sustained improvement over time with frequent data refreshes and enhancement of existing data.
Using EHR Data to Identify Wisconsin ZIP Codes with High Percentages of Adults at Risk for Severe Complications from COVID-19
In the midst of the COVID-19 pandemic, the ICTR Neighborhood Health Partnerships Program, in partnership with HIP, leveraged the existing NHP data infrastructure to rapidly provide public health decision makers in Wisconsin with county-level maps and reports that identified ZIP codes with high numbers or percentages of people at risk for severe complications from COVID-19. The poster outlines the methodology, results, and implications for the reports/maps which included the severe COVID-19 risk factors that were published by the Centers for Disease Control and Prevention (CDC) and could be reliably identified in electronic health record data.
