The Wisconsin Collaborative for Healthcare Quality released the Rural Wisconsin Chronic Disease Toolkit, and updated the Toolkit for Improving Hypertension Care & Outcomes and the Toolkit for Improving Diabetes Care & Outcomes.
The new Rural Wisconsin Chronic Disease toolkit was developed in conjunction with the Rural Wisconsin Health Cooperative (RWHC) to provide healthcare organizations providing primary care to rural populations with an up-to-date compendium of rurally relevant best practices that have been shown to be effective in improving chronic disease outcomes.
The updated versions of the Improving Hypertension Care & Outcomes and Improving Diabetes Care & Outcomes toolkits contain new resources and materials that healthcare organizations can use to improve outcomes for patients with hypertension and diabetes. The recommendations in the updated toolkits arose from steering teams composed of clinical, quality, and operational leaders across healthcare provider organizations and subject matter experts from other key stakeholders.
