Role of Telehealth Use in Chronic Care Management and Disparity Reduction Among the Aging Population

Telehealth use has rapidly increased in primary care since the outbreak of the COVID-19 pandemic. Chronic care management programs within health systems increased their use of telehealth to provide timely care and prevent exposure to COVID-19. With the expansion of telehealth use and its potential to overcome barriers to care access, health systems and patients must have real-world evidence of the benefits of telehealth use on health outcomes and disparity reduction to optimize telehealth use in multiple chronic conditions management.
